Chapter I – Privacy Policy
Under Article 13 (1) and (2) of the General Regulation of the European Parliament and the Council (EU) 2016/679 of April 27, 2016, on the protection of natural persons about the processing of personal data and the free movement of such data and repealing Directive 95/46/EC (General Data Protection Regulation), we inform you that:

(2) The co-administrators of your data are Biomed Neurotechnologie sp. z o.o. Al. Armii Krajowej 2/7, 50-541 Wrocław, NIP: 8971773878 and Robert Kozłowski Biomed Centrum Rehabilitacji Cognitive and Neurotherapy, Al. Armii Krajowej 2/13, 50-541 Wrocław, NIP: 8981884711.
(3) The provision of personal data by you is voluntary, however, failure to provide data qualified as necessary for the performance of the contract to which the data subject is a party prevents the conclusion and performance of the contract, and thus the performance of the desired service.
4 The administrator obtains and processes the following personal data:
(a) name
(b) mailing address
(c) telephone number
(d) e-mail address
(e) Company name – (if applicable)
(f) Tax Identification Number – (if applicable).
(g) business address – (if applicable)
(h) and other data necessary to complete the contract.
5. personal data was obtained from the purchase form on the Portal or e-mail correspondence and will be processed for the following purposes:
(a) performance of services, including for the performance of the concluded contract and sale of the Service,
(b) ongoing contact resulting from the concluded contract or from actions taken to conclude the contract,
(c) to enable the provision of electronic services, including the handling of requests,
d) direct marketing and promotion of own products and services,
(e) answering questions sent, conducting correspondence,
(f) to fulfill legally binding obligations on the Data Controller in connection with the need to keep accounting evidence,
(g) defense against claims and assertion of legitimate interests of the Data Controller, until the expiration of the claim period by the Civil Code.
Legal basis: art. 6 (1) RODO
(6) The controller shall process the data by the law, collect them for designated legitimate purposes, and not subject them to further processing incompatible with those purposes. Data are collected only to the extent that is adequate, necessary, and necessary for the purposes for which they are processed. The administrator does not process special categories of personal data. Provision of data is voluntary but necessary to provide services.
7 Your data will not be transferred to third countries or international organizations.
8 The recipients of your data are:
(a) persons authorized by the Administrator and entities providing services to the Administrator,
b) subjects whose data processing is necessary for the performance of the concluded contract, or its conclusion,
(c) entities entrusted by the Administrator with the processing of personal data (processors) based on contracts concluded,
(d) entities entitled to receive them under mandatory provisions of law.
(9) The Administrator shall make every effort to protect the personal data of customers and users from unauthorized access by third parties, and in this regard shall apply organizational and technical security measures corresponding to the given risk of personal data processing. The Administrator does not make personal data available to any unauthorized recipients.
(10) The Administrator shall keep personal data for the period necessary for the processing and the contract and no longer than the period specified in the applicable laws in this regard and no longer than necessary for the legitimate interests pursued by the Administrator. In connection with the marketing of products and services offered by the Administrator, the data are kept until the withdrawal of consent.
11. in connection with the processing of your data, you have the right to:
(a) access to the content of the processed data (Article 15 RODO),
(b) rectification of processed data (Article 16 RODO),
(c) erasure of processed data (Article 17 of the RODO),
(d) restriction of data processing (Article 18 RODO),
(e) portability of processed data (Article 20 RODO),
(f) object to the processing of data (Article 21 RODO),
(g) complain about the connection with the processing of personal data by the Controller to the PUODO (Article 77 of the RODO).
12 The data provided to us is stored and processed by us by appropriate security measures that meet the requirements of Polish law. All our employees are required to comply with our Policies and Procedures related to confidentiality, security, and privacy.
